Has made a clear, meaningful difference in the health, happiness, or overall well-being of those they support.
 
4. Reliability
Known for being dependable, consistent, and a trusted part of the care team.
 
 
How to Nominate
 
  • Write a short description explaining how your nominee meets the above criteria.
  • Include real examples or stories that show their outstanding caregiving in action.

Recognition
 
The selected recipient will be honored with C-Care’s 2025 *Healthcare Worker of the Year* award and receive a commemorative certificate of recognition.
